{"text":"NH$ Confidential: Personal data abotsd n patient \nNHS, - \nLanarkshire \nFollowup: \nWithGP \nThis 42 year old lady presented to ambulatory care with worsening lethargy and shortness of breath. \nOver the past 2 weeks she has been unable to climb the stairs having to stop 3 times due to lethargy. \nFamily have commented that she looks pale and that her oral intake has been reduced. \nShe was seen by the GP and bloods were taken 2 days ago. She denies any chest pain, she has a dry \ncough which is long standing and no haemoptysis. She denies any nausea or vomiting and eats a varied \ndiet. Her fast bowel movement was a brown stool with no blood and there has been no change in her \nbowel habit. She has no urinary symptoms. Her last menstrual period was 2 weeks ago which was not \nheavy with no clots. She has had some swelling and discomfort to the left calf whichhas now resolved, \nOn examination, she is thin and pale. Her weight is 45.5kg, Both calveswere soft and non-tender. NEWS \n~ scoreis 1with heartrate is 108. Her respiratory and abdominal examination were unremarkable and her \nheart sounds were pure, ' \nBloods show a haemoglobin of 61, an iron level of 2 and a ferritin of 4.6, ECG shows normal sinus \nrhythm and patient was transfused 1 unit of packed red cells and discharged home.
