{"text":"Page 3 of 5 \nCLINICAL INFORMATION \nHistory of presenting compla int \nPresenting complaint \nDescription: IDA\/increased heartburn \nComment: Dear Doctor, \nT would be grateful for you assessment and consideration of urgent scopes for this lady with a significant IDA. \nPatricia has a history of gastritis\/duodenitis and previously became anaemic due to this. In addition it appears \nshe was planned for a colonoscopy in 2022 for a positive Q-fit but she never arranged this. \nBloods were done last month due to new leg swelling and she was found to have Hb 69 and raised platelets. \nPatricia was admitted for assessment and had a transfusion. She feels a lot better following this but there \ndoesn't appear to be a plan for further investigations. Her Hb is now at 73 but her platelets are still raised. I \nhave started her on Iron. \nPatricta denies weight loss, haematemesis, swallow issues or bowel change. She has noticed her heartburn has \nworsened over the last few months and had increased her PPI to compensate. She is unaware of any other \nfood loss and reports regular periods. \n.
