She is a 40-year-old lady with nephrocalcinosis, presumed \ndistal type 1 tubular acidosis+\/- Hypocitraturia ~ the 24 hour urine citrate result is stilt outstanding. Miss Hunt has \nhanded in an early moming urine sample for pH sempling. Her PH was 6 ie she was not able to maximally acidify her \nurine, which again supgests a renal tubular acidosis. Miss Hunt has been well since I last met with her at clinic. Sheis \nmmhging her Sodium Bicarbonate tablets with most recent biochemistry showing both potassium and bicarbonate silling \nin a satisfactory range. However, her urea and creatinine had increased suggest that she is more mtravascular deplete. \nSha tells me she is drinking up to 3 litres of fluid per day. \nUtine dip today showed some non-visible hacmaturia, a smali amount of proteinuria and was positive for nitrates and \nJeucocytes, Although the urine looks stightly cloudy. Miss Hunt had no symptoms of urinary tract infection. Given the \nchange in biochemistry T have sent an\u2019MSSL sample to the tab for microbiology and I have repeated ULE's again \nurgently today. Tf there has been further deterioration, I will be in touch with Mr Hunt for probably antibiotics course and \nDanlbiu intravenous fluicds, I biochemistry is stable or improved today, thcn] will meet
