CKD G4 A3:(PCR 46 july 2025). \n2. Nephrocalcinosis. \n3. Non-steroidal anti-inflammatory overuse. \n4. Hypocitraturia. \n5. Renal tubular acidosis Type 1- for genotyping at next clinic. \n6. Anorexia. \n7. Iron deficiency anaetnia. \n8. Recent inpatient stay for sepsis\/septic shock secondary to pyelonephritis and urinary obstruction \nShe was due to come in for surgery on her renal stones but she failed to attend on the day- when| \nreviewed this in the pre-assessment clinic | found she had an NTproBNP level of over a 2000. She had \ngivenahistory of shortness of breath to the pre-assessment nurses and therefore | arranged an _ \noutpatient echocardiogram (3\/2) - this has picked up she is now in atrial flutter at a rate of 107bpm, \nunfortunately we were not alerted when she was in the hospital so | contacted her via the phone today. \nShe states she feels well and denied any shortness of breath, cough, haemoptysis, leg swelling or chest \npain. However | have manged to arrange for her to see her GP today as an emergency and will follow up \nthe outcome of the consultation, She tells me she is due back in clinic at the end of February so |
