He has a PMH of Severe COPD, inferior STEMI \n2023, ITU admission with diverticular perf 2025. \nDiagnosis: Oesophageal spasm + ?pulmonary hypertension for further Ix \nClinical course: \n- Admission bloods showed -ve trops, hyperinflated CXR with nil acute signs, raised D-dimer 342. Nil oxygen requirement \nduring admission. \n- Given the raised d-dimer was started on anticoagulation while awaiting CTPA \n- Given strong PPl and peptac + analgesia which improved symptoms, chest pain now resolved. \n- CTPA results - no definite acute pulmonary embolism but showed 'strands of low attenuation within the right middle lobe \npulmonary arteries are most likely to represent contrast mixing rather than thrombus. A chronic web is the other possibility.\u2019 \nThis was discussed with respiratory who advised pt should have OP V\/Q scan which has been requested on Trak and OP \nrespiratory follow up. He does not need anticoagulation at present. \n- Mr Norwood is now clinically well and can be discharged home with OP FU as above. \nPage 3 of
