Triggered by bending over and exertion, Feels a dull ache, \nstruggles to breath. Worse on inspiration, sore to breath. Radiates into the back. Lasted for 5 to 10 \nmins after rest. Feels he is having to stop at times, often slows down. \nRelevant Past Medical History \nCOPD. \nInvestigations \nECG ETT \nHeart Rate: 76 (bpm) ETT Done. \nSinus rhythm. Protocol: Bruce. \nNormal conduction. Ex. Time: 00:00. \nNon Ischaemic. \nClinical Findings \nBP (mmHg) 164\/102 \nSmoking Status Current Smoker \nDiagnosis and Recomendations \nAdditional discharge comments: It is my gut instinct that these symptoms are related to his COPD as they are worse on \nbreathing in. he does, however, have risk factors for IHD and it would have been nice to rule this out by ETT. \nHypertension precluded ETT today. \n| have given him a script for a trial of a GTN spray to use prophylactically and also for symptoms. | suspect if may not be \neffective for him, however, if it is and the BP is under control please prescribe, aspirin, atorvastatin and refer back for \nETT. \nPlease can you arrange to check his BP and prescribe an anti hypertensive as required. \nWorsening advice given. \nYours Sincerely,
